Emergency Feeding Plan Form
The Arkansas Department of Education Child Nutrition Unit (ADE CNU) has requested each school district to consider their plan should school nutrition staff test positive for COVID-19 and require quarantine. In the spirit of trying to assist with this effort, Arch Ford Education Service Cooperative has prepared this form to assist school districts in the development of their own local plans using ADE CNU guidance dated August 18, 2020.
What method of food preparation and service will be utilized for COVID-19 emergency feeding? The affected campus will:
satellite meals from another campus within the same school district.
satellite meals from another campus at a neighboring school district.
utilize staff from another campus within the same school district and continue to prepare meals on-site.
call part-time, on-call, or substitute help and continue to prepare meals on-site
utilize staff from the affected campus, i.e. office staff, principals, counselors, etc., and continue to prepare meals on-site
utilize community volunteers, i.e. churches or the local food bank, and continue to prepare meals on-site
Identify partners to the emergency feeding plan, i.e. neighboring school district, local churches. Consider adding names and phone numbers to this section.

Prepare five (5) to ten (10) days worth of emergency menus, with portion sizes for both breakfast and lunch, that can be utilized during a two-week period when school nutrition staff may be in quarantine. Upload the menu file here.
NOTE: Consider skill sets of the individuals preparing and serving meals in the absence of the school nutrition department and any storage requirements for foods to be ordered in advance and stored on the premises.

Identify inventory, food and non-food, already on the premises that is retained for the purpose of this emergency feeding plan. Plan for at least three (3) days of inventory on-site to start the emergency menu. Upload the inventory list here.
NOTE: Be sure to include the name of the food, the quantity on hand, and the location of the inventory.

Prepare a shopping list that can be used in the event of emergency feeding. This shopping list covers additional inventory that will be needed beyond the inventory already on hand and identified above. Upload the shopping list here.
NOTE: Be sure to include the name of the food, the distributor's item number and the quantity to purchase. Include distributor's contact and phone number.

The school district will notify the Arkansas Department of Health (ADH) in the event of a positive COVID-19 case and follow ADH instructions regarding quarantine. Will the school district notify the community?
NOTE: If yes, the school district will follow its COVID-19 plan for the method in which disclosure will take place and how privacy will be protected.

Once submitted Arch Ford Education Service Cooperative will retain a copy of the school district's COVID-19 Emergency Feeding Plan and return a copy to the school district for filing. Thanks for remaining diligent in mitigating COVID-19 risks.
