·         I agree to play the following 2025 football season with the above mentioned team, A member of the Central Midwest Football League hereby known as the CMFL from the date of form submission until November 1, 2025.

I agree to conduct myself in a professional manner on and off the field at all times. I agree to abide by all team and league rules and understand that if I violate any rule regarding conduct or violence I may be immediately expelled from the above mentioned team as well as the CMFL which houses the above mentioned team. Depending on the severity of the offense, legal action may be taken against me from both the CMFL and the above mentioned team for physical violence upon a living being, theft of property or property damage caused by me under the Law of the state in which the incident occurred. *

Most teams in the CMFL charge a player participation fee. I agree to pay my fees in full prior to the beginning of the 2025 season. I understand that my membership fee does NOT cover my uniform expense unless dictated otherwise by the team owner or legal representative of the team which I choose to play for.

Each team requires players to participate in fundraisers as a means to help offset the cost of team expenses. I understand that my failure to participate in these efforts may result in additional out of pocket expenses to cover the cost of uniforms.*

I understand that by signing this agreement; the above mentioned team owns my rights to play in the league. I understand that I cannot play for any outside team(s) or league(s) within a 150 mile radius without consent and written release from the above mentioned team and the CMFL Board of Directors. I also understand that if it is found that I have violated my agreement in any way I will be suspended for the remainder of the season.

I understand that as a paying player in the CMFL, I initially have the right to sign with any one team I choose. If I sign with a team and get released, I understand that I will NOT receive a refund, that I must select a team that has an available roster spot open before the final roster deadline. If I have not signed by the official roster deadline, I understand that there will not be any provisions to allow you to sign after said date.*

Section II: RELEASE OF LIABILITY AND ASSUMPTION OF RISK AGREEMENT.
·         I understand that I have signed up to play full contact adult amateur football in the Central Midwest Football League with the above chosen team, I KNOWINGLY AND FREELY ASSUME ALL SUCH RISKS AND FUTURE RISK THAT MAY ARISE FROM MY PARTICIPATION, both known and unknown, EVEN IF THE RISKS ARISE FROM THE NEGLIGENCE OF OTHERS. I assume full responsibility for my participation in the above mentioned team and the CMFL for the 2025 season and beyond.*


I willingly agree to comply with the stated and customary terms and conditions for participation of adult amateur football in the Central Midwest Football League (CMFL). If, however, I observe any unusual significant hazard during my presence or participation, I will remove myself from participation and bring such to the attention of the nearest official immediately and contact the League Commissioner and or League Owners.*

I, for myself and on behalf of my heirs, assigns, personal representatives and next of kin, HEREBY RELEASE AND HOLD HARMLESS The Central Midwest Football League officers, officials, agents, and/or employees, other participants, sponsoring agencies, sponsors, advertisers, and if applicable, owners and leaser of premises used to conduct the event (RELEASES), WITH RESPECT TO ANY AND ALL INJURY, DISABILITY, DEATH, or loss or damage to person or property, WHETHER ARISING FROM THE NEGLIGENCE OF THE RELEASES OR OTHERWISE, to the fullest extent permitted by law.*


I have read section II THE RELEASE OF LIABILITY AND ASSUMPTION OF RISK AGREEMENT, I fully understand it\'s terms and understand that I have given up substantial rights by agreeing to the terms and submitting this form freely and voluntarily without any inducement.*

Section III: EMERGENCY CONTACT INFORMATION.
·         If I get injured to the point where I am incoherent or unconscious and have to be transported to an off-site medical facility; I hereby grant my consent to be transported to a local medical facility (Emergency Room) where I can be medically treated by a licensed physician.*

Section IV: GENERAL CONDUCT.
·         I agree to conduct myself in a professional manner at ALL TIMES! I will refrain from conduct that will embarrass my organization as well as the Central Midwest Football League and its members. I understand that the CMFL plays the bulk of its games at high school facilities therefore the use of alcohol, tobacco and narcotics are strictly prohibited on school property which includes personal vehicles while on the property (parking lot, locker rooms, etc). The use of the aforementioned substances prior to a game, practice, or event will result in an automatic ban from the Central Midwest Football League.*

As a member of the CMFL, I agree to exhibit respect for fellow teammates, coaching staff, fans, and sponsors. Excessive arguing and foul language will not be tolerated and will be grounds for immediate suspension up to one complete season from the Midwest Football League pending the outcome of a thorough investigation.*

I agree to respect the property of the facilities that houses any and all CMFL teams. I understand that any form of theft or vandalism will not only result in your immediate ban but will result in your arrest and prosecution in the town where such events have occurred.*

I agree to refrain from any unacceptable behavior on the sidelines, including any action that will embarrass or prevent a sponsor from working with your current organization. This includes excessive profanity, excessive yelling and the removal of equipment during a game. This also includes fraternizing with the fans. Players will not conduct conversations in a negative manner with fans during or after the game, this will also include: pre-game warm ups, introductions, during the game and half-time. Players may never invite family for friends to sit and converse on the sideline during a game.

Finally, players may not go beyond the 30-yardline at any time during a game. Every player must respect the officials request to stand behind the designated area. Any player or team found to be in violation of this rule will be reprimaneded.*

Maintain prompt and regular attendance at all practices, games, and team events. In the event, you will be late or absent it is your responsibility to notify your teams ownership as soon as possible prior to the start of the practice, game, or event. Players will also be responsible for making sure they obtain any information or assignments that they have missed. If you have a preexisting engagement that will prevent timely arrival at team practices, games, and events you must notify team ownership. Absences or excessive late arrivals will result in disciplinary action up to removal from your current organization.*

Uniforms provided to you are the property of the organization which you have signed your player agreement. All jerseys must be returned to the organization by seasons end unless other arrangements have been made in writing stating you do not have to return said equipment by the organization which holds your player agreement. Failure to return property to the organization will result in legal action and a permanent ban from the Central Midwest Football League for theft of property. All players must be in complete matching uniform in order to participate in games. All jerseys shall cover back plates.*

Most of the games in the Central Midwest Football League will be played at a High School, College or Public facility; therefore firearms are never permitted in or around any organizations stadium. If player is caught with fire arm on or near sideline, player will be banned from league. Anyone caught branding a firearm will be immediately banned from the Central Midwest Football League forever and legal action will be taken against you. *

Players will be forbidden from excessive cursing and using the \"N\" word or any other racial remark where officials, fans and fans of an opposing team will accuse the organization of having negative behavior. Racism will NOT be tolerated in the Central Midwest Football League and any player, owner or coach that is found to be in violation will be suspended from the Central Midwest Football League.*

I understand most teams in the Midwest Football League charges a Player Participation Fee. Some teams charge more than others based on financial need. I understand that if I do not pay the required player fee by the team\\\'s due date, they can forfeit my ability to play in the Midwest Football League for the 2025 season. I understand that there will be ZERO extensions given so I must make the necessary arrangements to have my fees paid on or before the team\\\'s specified due date.*

At no point before, during or after the game is a player to approach an official in a negative or threatening manner. Only the Head coach or designated player(s) ie, team captain(s) are allowed to address the officials during the game. Any player that addresses an official that is not designated prior to the game or any player that addresses an official in a threatening manner will be subject to disciplinary action including but not limited to suspension or removal from the Central Midwest Football League.*

The CMFL is a ZERO tolerance league! Any player caught fighting (regardless who threw the first punch) will be suspended pending the outcome of a league investigation. Players that are not initially involved in the altercation must remain on the sidelines until order is restored. If anyone steps on the field or if a player(s) decides to participate in the fight or attempts to do bodily harm while breaking up a fight will be suspended from the Central Midwest Football League up to 5 games or termination of the rest of the season. Using a helmet as a weapon during a fight will result in a permanent season suspensions from the CMFL and legal action may be taken.*

Fraternizing with fans during a game is a violation of the Midwest Football League\\\'s Fraternization Policy. I understand that if I am caught fraternizing with fans while acting as a member of the CMFL during pregame, half-time or any portion of the game while on the sidelines, I will be immediately suspended for no less than 2-games.

Any coach or player that serving suspension cannot be at any games home or away till suspension is completed.*

·Violation of the Code of Conduct includes, but is not limited to, the use of vulgar language, disorderly conduct, violence, threats of violence or any other act that interferes with the orderly operation of a scheduled event by the Central Midwest Football League. *
