MINOR (CHILD) PHOTO RELEASE FORM

The following form gives official permission to Mineral Baptist Church on behalf of the parent/guardian of the Upward participant listed below to use photographs/video from the Upward Sports season for any legal use, including but not limited to: publicity, copyright purposes, illustration, advertising, and web content.

Furthermore, by completing this form, the signee understands that no royalty, fee or other compensation shall become payable to them or their family by reason of such use.
