This document is to ensure that employee’s funds are electronically deposited into their bank accounts. Employees must include an active account number along with a bank electronic routing number to the financial institution where the funds will be deposited. This information is completely confidential and should be treated with the utmost discretion on behalf of both the employer and employee.

Note: We do not recommend submitting Cash App or any other third party money transfer account details. It is easy to defraud individuals using Cash APP or any third Party money transfer Apps, Banks are considered most secure. ADP only recognizes actual banks.

The employee hereby authorizes LEADERSHIP, EDUCATION & ATHLETICS IN PARTNERSHIP to send credit entries (and appropriate debit and adjustment entries), electronically or by any other commercially accepted method, to my (our) accounts indicated below and to other accounts I (we) identify in the future (the “Account”). This authorizes the financial institution holding the account to post all such entries. I agree that the ACH transactions authorized herein shall comply with all applicable U.S. Law. This authorization will be in effect until the Company receives a written termination notion from myself and has a reasonable opportunity to act on it.

By electronically signing this document, you are authorizing LEAP to complete the direct deposit.
