L&S Travel Cancellation Form

Reservations are subject to cancellation as outlined in the terms and condition you agreed upon when accepting your estimate and making your deposit on your invoice. These terms and conditions can be found by reviewing your invoice. 

RESERVATION CANCELLATION PROCESS: A signed cancellation form is required from all adult members in the room.  Cancellations completed to any other avenue will not be processed. Upon receipt of the cancellation submission, you will receive the L&S Travel Cancellation Form to read, review, and sign. Upon receipt of the completed Cancellation Form, L&S Travel will process your cancellation. If any refund is due as outlined in the terms and conditions of your invoice, it will be processed within 90 business days and returned to your original form of payment unless otherwise stated. 

If you choose to cancel now and at a later date there are travel waivers that affect your date of travel the tour operators/hoteliers do not go retroactive and refund reservations that were previously canceled. 

CANCEL ANYTIME CLAIMS: All requests for service under the policy must be filed directly with the travel insurance provider, in accordance with the policy terms and conditions, which you the passenger are responsible for reviewing upon receipt of your travel protection policy. L&S Travel is not able to give advice with regard to possible cancellations and any associated claims processing. All queries regarding cancellation, penalties, coverage should be directed to your particular travel insurance provider. 

Please note that the travel insurance provider may not be allowed to discuss your claim with L&S Travel due to privacy laws (e.g. HIPAA). Accordingly, you acknowledge that L&S-T cannot be involved in any aspect of your claim/request for service. 

Passenger(s) acknowledge and agree that L&S-T has no control over the travel insurance provider or its coverage decisions, and as a result L&S-T is not responsible for and shall not be liable for policy coverage, claims processing, or the denial of any claims. 

CANCELLATION POLICIES: All cancellations or no-shows are subject to penalties imposed by the supplier. All booking cancellations, transfers to another Agency, or and/or no-shows will be  considered a cancellation, subject to a non-refundable $150 per occurrence, per person, fee imposed by L&S Travel. Upon making the booking, Passenger(s) hereby accept & acknowledge liability for all cancellation fees imposed by L&S-T and the supplier. Cancellation submission will be accepted at any time completing this form. Current day processing cut off time will be 5:00 PM EDT each normal business work day. Contact your L&S Travel consultant for the corporate business hours of availability. Cancellation documentation during normal business hours *and* prior to daily processing cutoff will be processed on that business day. Cancellations received after cutoff will be processed on the next business work day. Please note: Current business hours are 8:30 am to 6:00 pm, Eastern Standard Time / DST, Monday through Friday. The business is closed Saturday, Sunday, all nationally recognized holidays, and at the discretion of the owner for internal business requirements. Each cancellation will receive an email confirmation when complete. Cancellation once final payment date has passed is within full penalty, meaning final payment in full is due on the reservation. If you process your cancellation after final payment date and a late fee has been assessed, the late fee must be paid prior to the cancellation being processed by L&S Travel.  Payment will be due in full within 48 business hours to avoid legal & collections actions. All collection agency fees are the responsibility of all guests listed on the reservation and/or invoice, regardless of occupancy cancellation circumstances.   

AIRLINE TICKETING: All L&S Travel, LLC Air Travel reservations on behalf of the clients are classified as 100% non-refundable by the airline partner.  The guest assumes all rights, responsibility and liability by the airline partner. The guest assumes all rights, responsibility and liability for any airline carrier 100% non refundable  ticket penalty or fee, and indemnifies L&S Travel, LLC., and their partner/suppliers of any responsibility or liability whether resulting in monetary, legal, or compulsory resolve.   Wedding deposits, wedding/group booking deposits (including individual reservations under a wedding/group),  and Insurance are 100% non refundable under any circumstance. 

AIRLINE CANCELLATIONS: Airline tickets are non refundable and non transferable.  In the event airline flight cancellation, you will receive no refund for any funds paid towards the airline portion of your reservation.  Refer to the airline policy or contact the airline customer service group for further clarification of refund policies related to a specific flight or flight segment. All L&S Travel booked airline ticket cancellations with a reduced deposit require payment of the airfare in full as a prerequisite for complete cancellation.  Occasionally airlines change flight schedules & may even cancel flights entirely. These changes are beyond L&S Travel, LLC..’s control. In this event, L&S Travel, LLC. will strive to assist the client(s) with finding best possible alternate option(s). If the flight time change is minor (2 hours or less) the airline will automatically make the changes (airline policies/rules entitle them to legally do so.). L&S Travel LLC. will notify you or major changes. 

Please Note: there may be additional costs to re-book a new flight should the airline cancel or make a major change to your original flight itinerary. L&S Travel, LLC. is NOT responsible for any additional costs or fees imposed by the airlines. 

SEVERABILITY: If any provision of this agreement shall be unlawful, void, or for any reason unenforceable, then that provision shall be deemed severable from this agreement and shall not affect the validity and enforceability of any remaining provisions. 

CHARGE BACKS: If client opts to request a chargeback from their credit card company in order to avoid cancellation penalties agreed upon in terms and conditions listed on their estimate/invoice, L&S Travel will charge a $500 for each dispute received

NON DISPARAGEMENT: L&S Travel encourages our clients to provide us feedback on our services along with our suppliers services. Clients outlined above agree that they will not disparage the company or any of its officers. directors, or employees. For purposes of this Section, "disparage" shall mean any negative statement, review, whether written or oral, about L&S-T and its employees, officers, directors. Clients receiving service acknowledges that the actual damages likely to result from breach of this section "NON DISPARAGEMENT" are difficult to estimate on the date of this agreement and would be difficult for L&S Travel to prove. The parties’ intent that client's receiving services payment of Liquidated Damages amount of $50,000 per instance would serve to compensate L&S Travel for any breach by clients receiving services of its obligations under this section "NON-DISPARAGEMENT", and they do not intend for it to serve as punishment for any such breach by clients receiving services. 

LIMITATION OF LIABILITY: Without limitation, Passenger(s) assume the risk of, and agree that L&S Travel is not liable for any damages arising from or related to any act of God or public enemies, arrest, restraints of any government or rulers of people, piracy, war, revolution, extortion, terrorist activity, threatened or actual rebellion, political upheaval, civil unrest, riots, fire, lockouts, explosion, collision, weather conditions, dangers incident to the sea, mechanical or construction failures or difficulties, diseases, local laws, abnormal conditions or developments, closure of airports/seaports/hotels/train stations, carrier or supplier logistical problems, computer problems stranding, food or water poisoning, illness, grounding, perils of the sea, rivers, canals, locks or other waters, perils of navigation of any kind, theft, accident to or from machinery, boilers, or latent defects even though existing at commencement of the trip, desertion or revolt of crew, or lost/damaged/delayed luggage. 

By signing below, we authorize L&S Travel to cancel our reservation and have read, understand, and agree to the terms listed above.
