Annual Grant Recommendations
For Annual Grant Recommendations, the grant recommendation section on the Change of Information Form must be submitted annually. For Recurring Grant Recommendations, the Foundation can rely on your recommendation indefinitely although you may change your Recurring Grant Recommendations by completing a revised grant recommendation section on the Change of Information Form.
In the event that any one or more of the selected charities either ceases to operate or loses its status as a qualified donee then the Board of Directors of the Veritas Foundation may in their discretion distribute the amount mandated by the then current granting policy of the Veritas Foundation to such other qualified donees providing they are subject to approval by the Board of Directors of Veritas. In the event that I do not provide any grant recommendations or the recommendations are not sufficient after being contacted by the Veritas Foundation then the Veritas Foundation shall disburse the funds required according to the aforesaid granting policy. I hereby confirm that I, my family and those not at arms-length from me, will not receive any benefit or advantage from any of the grants that I am recommending. As well none of the grants are to fulfill a legally binding pledge agreement.

Part 6: Deed of Gift
I/We hereby give cash and/or securities absolutely to the Veritas Foundation (the "Foundation").
This application form is accompanied by the Program Guide, which outlines the terms and conditions of opening an Account with the Foundation Please read it carefully and keep it for future reference.
I/We acknowledge that each donation, whether cash or securities, will be deemed to be a separate donation (the "donation") and will be liquidated and the proceeds will be used to purchase units of the Veritas Foundation Giving Fund and will be added to and form part of my Veritas Foundation Giving Account (the "Account").
I/We acknowledge that the Foundation grants at a minimum rate of 4% per annum. Grants will be paid out of the funds held in the Account. The actual grant amount is based on the Account's market value on December 31st of each year, except in the first calendar year of the account being open, when no grants will be made from the account.
I/We acknowledge that the Foundation shall be unrestricted in its ability to disburse part of or the entire balance of the Account and I acknowledge that the Foundation may consult the individual or entity then responsible for making recommendations to the Account about recommendations to the Account about whether they recommend granting more or all of the assets in the Account, but that the Foundation is not bound to do so.
I/We understand that the Board of Directors of the Foundation has the fiduciary obligation of managing the assets in the Foundation. I/We understand that the Foundation has the sole and final authority to select the investment managers, monitor their performance and change the asset allocation and investment management mandates as necessary and in compliance with all applicable laws.
I/We understand that
any donation represents an irrevocable gift to the Veritas Foundation
, a registered charity, and is not refundable to me for any reason. I/We acknowledge that up to 100% of this donation shall be deemed to be an endowment fund. The Veritas Foundation may apply for its charitable purposes such portion of the income, capital gains and original capital of the Fund (whether realized or accrued) as is permitted by this agreement and the
Income Tax Act (Canada)

The Veritas Foundation may use my information to process gifts, administer delivery of charitable services, establish, maintain and manage our relationship, including set up and management of the Fund and maintenance of an accurate record my involvement, provide information regarding the operations of the Fund and offer further opportunities to give; verify my identity and protect against fraud; satisfy regulatory obligations and other legal requirements; and to create statistics about our operations and understand the current and future needs and preferences of Donors.
In administering the charitable services of the Veritas Foundation, we may provide your personal information to other persons: where the other parties are grant recipients and you have consented to be recognized; where the other parties are our third party service providers, suppliers or agents who assist us in providing our services; and where we are required or permitted to do so by law.
