A charitable donation receipt will be issued for the fair market value of all donations received by the Veritas Foundation

PRE-AUTHORIZED CHEQUING TERMS AND CONDITIONS

By signing this form, you (the bank holder(s)) hereby waive any pre-notification requirements as specified by sections 15(a) and (b) of the Canadian Payments Association Rule H1 with respect to pre-authorized debits.
You authorize the Veritas Foundation to debit the bank account provided for the amount(s) and in the frequencies instructed.
This donation will be considered a Personal Pre-authorized Debit (PAD) by Canadian Payments Association (CPA) definition.
You have certain recourse rights if a debit does not comply with this agreement.  For example, you have the right to receive reimbursement for any debit that is not authorized or is not consistent with this pre-authorized debit agreement.

You confirm that all persons whose signature are required to authorize transactions in the bank account provided have signed this agreement.

You may change these instructions or cancel this plan at any time, provided that the Veritas Foundation receives at least 10 business days notice by phone or by mail.  To obtain a copy of a cancellation form or for more information regarding your right to cancel a pre-authorized debit agreement, please consult with your financial institution or visit the Canadian Payments Association website at www.cdnpay.ca.  You agree to release the financial institution of all liability if the revocation is not respected, except in the case of gross negligence by the financial institution. You agree that the information in this form will be shared with the financial institution, in so far as the disclosure of this information is directly related to and necessary for the proper application of the rules applicable for pre-authorized debits.
You acknowledge and agree that you are fully liable for any charges incurred if the debits cannot be made due to insufficient funds or any other reason for which you may be held accountable.
You have requested this application form and all other documents relating hereto to be in English.
You acknowledge that you have read the Program Guide and agree to all the terms and/or conditions described therein.  
You understand that any donation, including a donation made through a PAD Agreement, represents an irrevocable donation and is not refundable.

AUTHORIZATION

DEED OF GIFT

I/We hereby give cash and/or securities absolutely to the Veritas Foundation (the "Foundation").

I/We acknowledge that each donation, whether cash or securities, will be deemed to be a separate donation (the "donation") and will be liquidated and the proceeds will be used to purchase units of the Veritas Foundation Giving Fund and will be added to and form part of my Veritas Foundation Giving Account (the "Account").

I/We acknowledge that the Foundation grants at a minimum rate of 4% per annum.  Grants will be paid out of the funds held in the Account.  The actual grant amount is based on the Account's market value on December 31st of each year, except in the first calendar year of the account being open, when no granting amount will be allowed.  

I/We acknowledge that the Foundation shall be unrestricted in its ability to disburse part of or the entire balance of the Account and I acknowledge that the Foundation may consult the individual or entity then responsible for making recommendations to the Account about recommendations to the Account about whether they recommend granting more or all of the assets in the Account, but that the Foundation is not bound to do so.

I/We understand that the Board of Directors of the Foundation has the fiduciary obligation of managing the assets in the Foundation.  I/We understand that the Foundation has the sole and final authority to select the investment managers, monitor their performance and change the asset allocation and investment management mandates as necessary and in compliance with all applicable laws.

I/We understand that any donation represents an irrevocable gift to the Veritas Foundation, a registered charity, and is not refundable to me for any reason.  I/We acknowledge that up to 100% of this donation shall be deemed to be an endowment fund.  The Veritas Foundation may apply for its charitable purposes such portion of the income, capital gains and original capital of the Fund (whether realized or accrued) as is permitted by this agreement and the Income Tax Act (Canada) from time to time.

Your Privacy

The Veritas Foundation may use my information to process gifts, administer delivery of charitable services, establish, maintain and manage our relationship, including set up and management of the Fund and maintenance of an accurate record my involvement, provide information regarding the operations of the Fund and offer further opportunities to give; verify my identity and protect against fraud; satisfy regulatory obligations and other legal requirements; and to create statistics about our operations and understand the current and future needs and preferences of Donors.

In administering the charitable services of the Veritas Foundation, we may provide your personal information to other persons: where the other parties are grant recipients and you have consented to be recognized; where the other parties are our third party service providers, suppliers or agents who assist us in providing our services; and where we are required or permitted to do so by law.
