NOTICE OF PHOTOGRAPHY, VIDEO, & BROADCAST

This event includes photography, videography, and live Internet broadcast.

Your attendance grants the Epilepsy Foundation, its employees or agents, the right to take photographs, video, and live stream of your participation (adult and minor participants as well as Parent(s)/Guardian(s) of minors involved).

You transfer and grant the Epilepsy Foundation exclusive rights to use and to authorize others, including but not limited to Foundation affiliates, media outlets, production facilities, and the like, to use all or any part of your interview/photograph/video or film likeness, regardless of the medium by which it is recorded or broadcast.

By your attendance, you release the Epilepsy Foundation and its CEO, its directors, its affiliates, members, trustees, officers, employees, as well as its agents, from any and all claims, demands, causes of action and suits, including but not limited to, claims for invasion of privacy, defamation, breach of contract, or other breach of duty arising out of or in connection with the use of this interview, photograph, video, or live Internet broadcast. 

Attendance grants rights to exhibit this work in print and electronic form publicly or privately and to market and sell copies. There will be no financial or other remuneration for recording or broadcast, either for initial or subsequent transmission or playback.

 Specifically for participating minor(s):

1. The Parent(s)/Guardian(s) of minors participating in this event warrant having read and understood this release agreement and warrants being the Parent or legal Guardian of the minor, as well as being of legal age and competency and with every right to enter into an agreement on behalf of the minor and self.

2. The Parent(s)/Guardian(s) agree that the provisions contained herein shall be binding upon the Parent(s)/Guardian(s) and minor(s) as well as their collective successors and predecessors, legal representatives and assigns.
