Hospital keys are assigned to employees requiring access to the hospital. Keys are not distributed to employees until after their probationary period has been completed.
I understand that It is the employee's sole responsibility to report a lost or stolen key to the Hospital Administrator or Medical Directors immediately upon discovery. Loss of a key creates a security risk, as well as a resulting expense if re-keying is needed as a result of a lost or stolen key. The expense of re-keying the hospital (approximately $250) may fall on the employee losing their key.
I understand that If a key is not returned upon employment exit or termination, a $250 fee will be deducted from the employee's final paycheck.
I currently hold a key and understand, as well as agree to, the policy above.
In addition, I also have been shown how to use the alarm system of my assigned hospital.
I understand that the alarm code that is assigned to me will not be shared with another individual that is not management. I understand that I could be held personally responsible for any events that occur if/when my alarm code is used to commit a crime.
