Release, Waiver and Discharge
Please read this document carefully before signing where indicated. Your signature confirms that you have read, understood and accepted this release and discharge. In consideration of my participation in the program and any related tryouts, training, as a participant participation, I hereby release, waiver and discharge GTA Lions and its coaches, officers, employees and voluntary officials, of and from all claims, demands, damages, costs, actions, and causes of action, in respect to death, injury, loss of damage to my person or property, arising from participating in the tryout Program or use of any Athletic facilities.

Collection and Use of Personal Information: I hereby, give consent to authorities to utilize information gathered from my tryout records, pictures, and my training for the purpose of assessing my soccer progress, image promotions and athletic eligibility. I further acknowledge that any information gathered may be used for statistical and promotion purposes. . I also agree that the Scarboro Ath/GTA Lions may release my name and sports picture to the media or use them in various promotional publications, including the display of such on-website applications or marketing materials.
