I/We also understand the inherent risks associated with land transport and agree that MD Pet Services Sdn Bhd, its employees and representatives will not be held responsible for any loss or injury to my pet(s) during transport.

I/We agree to exempt MD Pet Services Sdn Bhd, its employees and representatives from any liabities or legal proceedings arising from such transport.

I/We understand that insurance for travelling pet(s) is not available through MD Pet Services Sdn Bhd.
