Swanson Pool Group Reservation  Request Form

A minimum of 10 participants is needed to receive the group rate. 

A certificate of insurance naming the "St. Charles Park District as Additionally Insured" 
is required for all non-PDRMA groups.

Terms and Conditions:

1.  User Group shall have the use of the facility for the date(s) and time(s) stated herein, and subject to the facilities group or rental fee as provided herein.

2.  User Group shall fully comply with all St. Charles Park District rules, regulations, and ordinances in connection with the use of the facility.

3.  User Group shall fully comply with all facility rules.

4.  Each person entering facility must pay the appropriate admission fee.

5.  User Group is solely responsible for the action of any member of User Group and shall provide adequate adult supervision (age 16 or older) of group at all times.

6.  User Group Supervision Ratios shall minimally meet the following:

7.  User Group must bring roster with names, address and phone number for each group member.

8.  All swimmers are subject to a swim test. User Group must be aware of the group members swimming abilities.

9.  All adult supervisors and counselors with the User Group must be readily identifiable as being with that group for emergency contact purposes.

10.  The St. Charles Park District shall provide lifeguard staff for users of the facility. User Group fully understands and agrees that lifeguards are not responsible for supervising User Group. Lifeguards are responsible for enforcing rules and regulations and responding to emergencies.

11.  User Group is solely responsible for the safety and security of any property brought to the facility. The St. Charles Park District is not responsible for lost, stolen or damaged personal items.

12.  The St. Charles Park District retains the right to cancel the agreement at anytime and for any reason, including but not limited to inclement weather, misconduct of the group (or any members of the group), or for misuse of property, for purposes deemed necessary for public safety or preservation of property, if termination serves the interest of the Park District residents, or because the group has breached any of its obligations under this agreement.

This is a request only. A confirmation will be sent out once your request is processed, within 3 business days. The confirmation must be signed and returned. A certificate of insurance naming the “St. Charles Park District as Additionally Insured” is required for all non-PDRMA groups. Failure to provide these things can and will result in cancellation of your group reservation.
