By checking this box I agree to provide a projected budget for proposed course(s)/program(s). Include what your Association will contribute, cost for members & non-members if applicable
By checking this box I agree that our Local Association will submit a report within 30 days after completion of the course(s)/program(s), describing and confirming how funds were used
By checking this box I agree that our Local Association will submit a 3 minute video after the program/event, sharing the content and its outcome
