CONSENT FOR RETINAL PHOTOS 
As part of your eye exam, the optometrists here at New River Health recommend a special diagnostic procedure called retinal photography.  This procedure consists of taking a picture of the back of your eye for your permanent record.  This is not an ultrasound or an x-ray, but a very simple picture that will allow us to better assess and monitor your eye for changes now, and in the future.   Our doctors hope that you will take advantage of this technology and would like everyone to do this every year, or every other year at the very least, in addition to your routine exam. 

This procedure is non-invasive and will not touch your eye in any way.  It’s a matter of you looking at a light while the machine takes a picture.  An optometrist will review the photos with you during your exam.  This will allow the optometrist to observe even the smallest change from any previous photos and will allow us to provide you with a more thorough medical analysis of your eye health.
Our optometrists strongly advise and recommends this procedure as part of your exam if:
You are a new patient to this office
You have never had retinal photos in this office, or it’s been a year or more since your last photo
You personally have or have a family history of diabetes or elevated blood sugar
You personally have or have a family history of elevated eye pressure, glaucoma or macular degeneration
You have headaches or visual disturbances suggestive of a neurological problem
You have or have had any retinal disorder such as detachments, scars, floaters or flashing lights, veil over vision
You currently use or have a history of using blood thinners
You think you have cataracts or have a history of trauma to your eyes
You were told by your previous eye doctor of some changes in the back of your eye

Your digital images will be stored in your chart to be compared with images from past or future exams.
