SECTION A: ADVISORY NOTE TO STUDENTS (FORM 12)
PRIVATE EDUCATION ACT

This note is for prospective student:
You are strongly encouraged to thoroughly research on the private education institution (PEI) conducting the course before signing up for any course. You should consider, for example, the reputation of the PEI, the teacher-student ratio of its classes, the qualifications of the teaching staff, and the course materials provided by the PEI.<br><br>By signing and returning the Student Contract (the “Contract”), you agree to the terms and conditions which will bind you and the PEI, if you accept the PEI’s offer of a place in a course of study offered or provided by the PEI.<br><br>You should ask the PEI to allow you to read a copy of Student Contract. For your own protection, you should review all the PEI’s policies, and check carefully that you agree to all the terms of the Contract, including the details relating to each of the following sections, before signing the Contract:<br><br>a) The duration of the course, including holidays and examination schedules, and contact hours by days and week.<br>b) The total fees payable, including course fees and other related costs. <br>c) Dates when respective payments are due.<br>d) The refund policy in the event of voluntary withdrawal (by you) or enforced dismissal from the course or programme by (by PEI). <br>e) The dispute resolution methods available, and <br>f) Information about the PEI’s policies on academic and disciplinary matters.<br>g) The degree or diploma or qualification which will be awarded to you upon successful completion of the course<br><br><br>If you have any doubt about the contents of the Contract or if the terms are different from what the agent or the PEI have informed you previously, or advertised, you should always seek advice and/or clarifications before signing the Contract.
The portion below is to be completed by the signatory of Student Contract, i.e. either the student, or if the student is below the age of 18, his parent or guardian.

SECTION B: STUDENT DECLARATION
I declare that :
1a. I have/had registered with the Chartered Secretaries Institute of Singapore (“CSIS”) for the CGI-Chartered Governance Qualifying Programme (“CGI-CGQP”) examinations.<br><br>1b. Should I be unable to register for the CGI-CGQP examinations conducted by CSIS, I allow ACM to cancel my student registration for the intake.<br><br>2. I allow ACM to request and collect my personal data and examination records from CSIS for the purpose(s) of updating my student information and for audit checks conducted by the SkillsFuture Singapore Agency (“SSG”).<br><br>3. I fully understood that the CGI-CGQP preparatory course is offered on a per module basis and I am required to sign one (1) student contract (“Contract”) per registered module.<br><br>4. I shall inform ACM Staff by writing to students@acm.edu.sg immediately, if I wish to withdraw from the CGI-CGQP preparatory course in any period of the tutorial.<br><br>5. I am aware that the chartered secretary qualification will only be awarded to me when I have passed all the required modules under the CGI-CGQP.<br><br>6. All information which I have provided to ACM for the purposes of the registration for the preparatory course is true and accurate to the best of my knowledge, and shall be bound by the prevailing terms and conditions of the Contract.

PERSONAL DATA PROTECTION ACT DECLARATION FORM
Personal Data Protection Act - Student's Declaration :
1. In compliance with the Personal Data Protection Act 2012 (“PDPA”), Academy for Corporate Management (“ACM”) seeks your consent to collect, use and disclose your personal data in order to maintain the ACM registry of students and to disclose such personal data to ACM’s affiliated organisations where necessary (“Purposes”).<br><br>2. In some circumstances, you may have provided personal data relating to other individuals or organisations. You represent a warrant that you are authorised to provide their personal data to us and you have obtained their consent to the collection, use, disclosure and processing of their Personal Data in accordance with this Declaration.<br><br>3. ACM will collect, use and disclose your personal data to provide you with information on studentship, examination and activities such as upcoming events, seminars, workshops, conferences and training programmes organised by ACM and its affiliated organisations which may be relevant to you (“Services”).<br><br>4. ACM may also retain and continue to process your personal data for all purposes and services unless you request in writing to ACM at students@acm.edu.sg to withdraw your consent.<br><br>5. ACM reserves the right to update and amend the Personal Data Protection Policy from time to time. ACM will notify you of any amendments via announcements on its website at www.acm.edu.sg or any other appropriate means.<br><br>6. In the event that I have registered my Singapore telephone number(s) with the Do-Not-Call Registry and wish to withhold or withdraw my consent to ACM in respect of receiving telephone calls and/or SMS, I endeavour to provide sufficient notice to ACM of such as soon as reasonably practicable. I further agree to indemnify ACM against any financial penalties imposed by the Personal Data Protection Commission or any court of law in Singapore as a direct or indirect result of my failure to inform ACM of my registration with the Do-Not-Call Registry.
By agreeing to the Personal Data Protection Act - Declaration, my personal data collected in this form is processed, retained and used by Academy for Corporate Management (“ACM”) in accordance with the ACM Personal Data Protection Policy.
