Applicants Conditions of Acceptance

I agree to participate under the guidance and direction of National Youth Ministries and agree to honour and obey the OYM leaders and the leadership of the church I visit. I will abide by and respect the involvement guidelines above.

I understand that the number of those accepted into the program is limited and that a passport is required to travel overseas.

ASSUMPTION OF RISK

I understand that international travel and mission work involve inherent risks. I assume all risks associated with my participation in this trip.and release and hold harmless United Pentecostal Church of Australia Ltd, its directors, officers, employees, and volunteers from any and all liability, claims, demands, actions, or causes of action arising out of my participation in this trip.

MEDICAL CONSENT

In the event of a medical emergency, I authorize United Pentecostal Church of Australia Ltd and its representatives to seek and obtain medical treatment on my behalf. I understand that I am responsible for all medical expenses incurred and that I am encouraged to obtain travel medical insurance before departure.

CODE OF CONDUCT

I agree to abide by the rules and guidelines set forth by National Youth Ministries and United Pentecostal Church of Australia Ltd for the duration of the trip. I understand that failure to comply with these guidelines may result in my early return home at my own expense.
