Terms and Conditions: I, for and in consideration of the benefits derived from my participating in the Marshall County Youth Basketball program and/or Elevate MC, do hereby assume all risks and hazards incidental to such participation, including transportation to and from all activities, and do so hereby waive, release, absolve, indemnify and agree to hold harmless the Marshall County Youth Basketball program and Elevate MC, the Marshall County Board of Education, Marshall County Youth Basketball and Elevate MC directors, organizers, sponsors, supervisors, participants and persons transporting players or participants to and from activities, for any claim arising out of injury to myself. I also agree to carry medical insurance and be responsible for any and all expenses that may arise from my participation in any Marshall County Youth Basketball or Elevate MC event. I further agree to abide by any and all COVID-19 requirements set out by the Marshall County Board of Education and from each local school.

I have read the Terms and Conditions and agree to them. I also acknowledge that my likeness may be featured in promotions for the program and that I am not entitled to receive any royalties or other compensation in connection with such use.
