I have read and agree to the Hillnote Studio Policies, including the lesson cancellation and make-up lesson policy. I understand that Hillnote Music Studio charges semester fees rather than per lesson fees, and that I am responsible for paying the entire semester fee in full by one of the payment methods selected above. I understand that when I submit this registration form that I will be placed on the schedule with one of the days and times that I have selected above or agreed to after registering.

I authorize Hillnote Music to use photographs and videos of the student(s) listed above on this form publically to promote Hillnote Music. I understand that images and videos may be used in print publications, the Hillnote Music website and company social media accounts. I also understand that no royalty, fee or other compensation shall become payable to me by reason of such use.
